An Overview Of Fibroids

Fibroids are benign or non-cancerous tumours commonly seen in women of childbearing age. The risk of cancer is low.

They are made of fibrous tissues and muscle cells that can grow inside or outside the uterus (while remaining attached to the uterus).

Based on their location, the fibroids can be classified as:

Intramural Fibroids: Grow within  the uterus-in muscle wall

Subserosal Fibroids: Grow on the outer uterine wall

Submucosal Fibroids: Grow  into the cavity of the uterus

Pedunculated fibroids: These are submucosal or subserosal fibroids attached to a pedicle or a “thin stalk”.

There is no exact cause for their development, but high estrogen levels (female hormone) contribute to their growth, whereas low estrogen levels shrink fibroids. This is why fibroids usually stop growing after menopause.

Are fibroids dangerous?

In most cases, fibroids are asymptomatic. It can lead to missed diagnosis if no gynaecology checkup is done.

However, some symptoms include:

  • Heavy menstruation causing anemia, fatigue, dizziness, or pain
  • Big fibroids can cause frequent urination, urination incontinence, and back and leg aches as pressure effect. 
  • It can cause pressure on the ureter and cause slow kidney damage
  • If placed inside the cavity – it can cause infertility

Fibroids are diagnosed with routine gynaecological tests, vaginal examination, or ultrasounds via the vagina.

The option of treatment depends on many parameters like:

  • age 
  • number of children
  • desire to become pregnant later
  • number, size & position of fibroid
  • not interested to remove the uterus

Types of fibroid surgery

Depending on the diagnosis, risks and patient’s preference, the different types of fibroid surgery in Abu Dhabi include:

It is a non-surgical fibroid treatment. The embolisation aims to cut off the blood supply to fibroids; as a result, the fibroid cells die and shrink to some extent.

It is a procedure to shrink fibroid with lasers.

Endometrial ablation is the process of removing the uterus lining surgically in an attempt to stop excessive bleeding. This is symptomatic treatment only (but fibroids can grow).

It is a procedure to remove fibroids only. Myomectomy preserves the natural anatomy of the uterus.

This is a procedure for the removal of uterus only. It can be done with or without ovary removal.

Hysteroscopic removal of submucous fibroid is done when it’s small and type 0-1 submucosal fibroid – wherein a small camera is passed through the vagina and the fibroid is removed in pieces.

Types Of Hysterectomy Surgeries

Any size of the uterus,  small to large fibroids and single or multiple fibroids can be removed with any of the following types of hysterectomy.

The laparoscopic approach for all myomectomy cases has a very low risk of complications as compared to open surgery. The biggest fibroid removed by laparoscopy was of 1.2  kg without removing the uterus.

  • BY vNOTES technique (vaginal Natural Orifice Transluminal Endoscopic Surgery)-

 Recent advances in performing hysterectomy wherein the uterus is removed through the natural opening i.e vaginal canal. A small incision is made in the vagina and the uterus is taken out trans vaginally without leaving any cut on the abdomen.

  • By Non-descent Vaginal hysterectomy

Even the uterus which has not descended can be removed vaginally, which is the non-descent vaginal hysterectomy technique. It is also for women with a prolapsed uterus (when the uterus shifts down to the canal). 

Open myomectomy or hysterectomy can also be offered if the patient demands due to cultural thoughts.

Till date (in 17 years of practice), only four cases have been converted to open surgery for patient safety. Most cases of hysterectomies are done through a minimally invasive hysterectomy approach.

Fibroid Surgery Case Studies

Here open Myomectomy was done and the uterus was maintained.

The reason for choosing an open myomectomy was multiple small fibroids inside the cavity – submucosal, and the patient wanted to retain the uterus.

This patient presented with heavy menstruation, anemia, lower abdominal pain, and mass in the abdomen.

Note – as the number of fibroids increases, the risk increases.

 

This is a case of a fibroid uterus, the young patient had just mass in the abdomen like a pregnant uterus with no symptoms.

These fibroids should be removed as it causes pressure on nearby structures and damage slowly like the ureter and kidney.

As compared to the above case, this fibroid was bigger and weighed more. A big mass in the abdomen above the umbilicus.

Note –  though the fibroid is big, the laparoscopic approach is used to remove the fibroid only.

Open hysterectomy (patient request). Laparoscopy can be done in these cases. 

Older patients with multiple fibroids and symptoms and not interested to maintain uterus are requested open surgery.
